Data Subject Information Request Form

Pursuant to Article 11 of the Law on the Protection of Personal Data ("Law" or "KVKK"), Data Subjects have the right to obtain information about their personal data processed. Natural persons whose personal data are processed within the scope of the Law are defined as "Data Owner" and the relevant Data Owner may submit the following requests by filling out this form and applying in writing to our Clinic:

1. To learn whether your personal data is being processed or not
2.Learning personal data collection method
3. To learn for what purpose the personal data is processed and whether it is used in accordance with its purpose
4. To learn how long your personal data will be kept
5. To learn to whom and for what purpose the processed personal data can be transferred
6. To know the third parties to whom personal data are transferred domestically or abroad
7. To request correction of personal data in case of incomplete or incorrect processing
8. To request the deletion or destruction of personal data or, if transferred, to notify third parties
9. To object if the result of the processed data is unfavourable to him/her
10. In case of damage due to unlawful processing of personal data, to demand the compensation of this damage.

The Data Owner may submit their requests within the scope of their rights specified in Article 11 of the Law in writing or by using the registered electronic mail (KEP) address, secure electronic signature, mobile signature or the electronic mail address previously notified to the data controller by the data subject and registered in the system of the data controller or through this form or by e-mail to info@manderindent.com.

Points to be Considered by Data Subjects during Application When indicating your relationship with our clinic, specifying information such as the date of employment if you are a former employee, and which company if you are an employee of a third party company will help us process requests faster. Requests for information will be accepted through the addresses given above and will not be processed if notified to another address in writing or electronically.
